Instructions
for downloading membership forms:
1.
Click on the links below entitled “Membership
Form 1” and "Membership Form 2".
2.
Once the forms have appeared on your screen:
Click
on “File” at the top of the screen, then
click “Print”
- Or -
Click
the printer icon on the toolbar at the top of the screen.
3.
Once you have printed the forms, you can fill them out
and bring them to either center.
